Zusammenfassung

Ein Score ist der Versuch, eine komplexe klinische Situation auf einen eindimensionalen Punktwert abzubilden. Eine solche Reduktion verfolgt das Ziel, übergreifende Aspekte wie Schweregrad oder Prognose als Kombination einzelner Fakten objektiv zu fassen, um sie dann in unterschiedlichen Kollektiven vergleichend darstellen zu können.

Jeder Versuch, die individuelle Situation eines Patienten zu dokumentieren, stellt bereits eine Reduktion dar, denn sie beschreibt nur das, was wir heute messen können oder meinen, messen zu müssen. Jeder Laborwert, jede Röntgenaufnahme, jede Blutgasanalyse, jedes EKG ist ein kleines Stück Information, ein Mosaikstein im Zustandsbild des Patienten. Die Gesamtheit dieser Befunde und ihre Veränderung über die Zeit ist ein Versuch, diese Komplexität – in reduzierter Form – abzubilden.
