Zusammenfassung
Patienten mit Brandverletzungen sind angesichts der Schädigungen von Haut- und Schleimhaut, physiologischer, endokrinologischer und immunologischer Stressreaktionen und evtl. chemisch bedingter toxischer Schädigungen in besonderem Maße infektionsgefährdet. Infektionen sind die größte Bedrohung, zuvorderst Infektionen der Weichteile, aber auch als Folge von Fremdkörpern (Beatmung, Gefäß- und Blasenkatheter). Deshalb sind strenge Indikationsstellung, tägliche Überprüfung und sorgfältige Handhabung der Fremdkörper von besonderer Bedeutung. Standardisierte Arbeitsanweisungen (»standard operating procedures«), regelmäßige Schulungen und eine der Qualitätssicherung dienende Infektions-Surveillance sind neben ausreichend ärztlichen wie pflegerischen Mitarbeitern Voraussetzung für eine den Anforderungen entsprechende Behandlung. Die disziplinierte Befolgung aseptischer Verhaltensweisen auf der Basis einer individuellen Risikoanalyse ist entscheidend für den Behandlungsverlauf und damit die Prognose.
