Zusammenfassung

Ein Inhalationstrauma führt beim Brandverletzten zu einem verlängerten Intensivstationsaufenthalt und einer Steigerung der Sterblichkeit. Trotzdem gibt es bis heute keine befriedigende und allgemein akzeptierte Definition dieses Krankheitsbilds. Nach Darstellung der gängigen Diagnosekriterien und deren Wertung werden die grundsätzlichen pathophysiologischen Besonderheiten dargelegt. Neben der grundsätzlichen Therapie eines Inhalationstraumas mit Sicherung der Atemwege und Beatmung mit einem positiv-endexspiratorischen Atemwegsdruck (PEEP) wird ein besonderer Augenmerk auf die systemisch toxische Wirkung der unterschiedlichen Verbrennungsprodukte wie Kohlendioxid und Zyanwasserstoff gelegt und die Möglichkeiten der spezifischen Antidotgabe erläutert.
