Zusammenfassung
Komplizierte Harnwegsinfektionen treten bei strukturellen oder funktionellen Anomalien des Harntrakts auf und sind häufig durch Erreger mit hoher Virulenz und Resistenz bedingt, was eine effektive Therapie erschwert. Neben anatomischen Fehlbildungen gehören zu den Anomalien bzw. Risikofaktoren die Obstruktion, neurogene Blasenentleerungsstörungen, Restharnbildung, Infektsteine, bakterielle Prostatitis und eine Katheterdrainage, begünstigende Faktoren sind auch Diabetes mellitus oder eine Schwangerschaft. Diagnostisch wichtig ist es, die korrigierbaren Anomalien aufzufinden, die dann therapeutisch korrigiert werden sollten, ansonsten sind Antibiotika indiziert, ggf. auch eine Langzeit-Suppressionstherapie.
