Zusammenfassung
Neben der adäquaten Primärtherapie ist eine sachgerechte Nachsorge bei Patienten mit stattgehabtem Ulkus unabdingbar. Essenziell sind die regelmäßigen ärztlichen Nachkontrollen (Fußaspekt, Neuropathie- und Gefäßstatus) gemäß Diabetespass bzw. DMP (»Disease Management Program«). Daneben ist auf passendes Schuhwerk entsprechend der Zuordnung der spezifischen Risikogruppe (Praxisempfehlung der DDG) zu achten; dies betrifft die entlastenden Schuhzurichtungen ebenso wie Diabetesschutzschuhe mit Zurichtungen oder maßgefertigte orthopädische Schuhe. Eine fachpodologische Mitbetreuung kann hilfreich und notwendig sein – insbesondere, wenn Patient und Angehörige dies nicht mehr zu leisten vermögen. Bei den komplexen Umständen und den dargestellten Bedürfnissen des diabetischen Fußsyndroms ergibt sich die Notwendigkeit der multidisziplinären und multiprofessionellen Teambetreuung. Nicht zuletzt sind Schulung und Information das Patienten und seines Umfeldes notwendig.
