Zusammenfassung
Die klinische Ernährung spielt in der Kinderchirurgie eine größere Rolle als in der Erwachsenenchirurgie. Bei Kindern und Jugendlichen kommt neben dem Erhaltungsbedarf für die Organfunktionen und dem Bedarf für körperliche Aktivität zusätzlich der hohe Energie- und Substratbedarf für das Körperwachstum hinzu. Das gerade in den ersten Lebensmonaten enorm rasche Wachstum mit einer Verdopplung des Körpergewichtes bei reifen Neugeborenen in nur 4–5 Monaten nach der Geburt und bei Frühgeborenen sogar in nur 6 Wochen, und auch die rasante Differenzierung der Gewebe und Organe hängen von einer auf das Körpergewicht bezogen sehr hohen und ausgewogenen Substratzufuhr ab. Besonders im frühen Kindesalter kontrastiert dabei der hohe Energie- und Substratbedarf mit nur geringen körpereigenen Substratreserven sowie stark limitierten Kompensationsmöglichkeiten durch noch unreife metabolische und renale Funktionen. Auch kurzfristige Nahrungskarenzen in der prä- und postoperativen Phase werden oft nicht gut toleriert. Zahlreiche Studien belegen, dass die postoperative Prognose von der präoperativen Ernährungssituation abhängt. Bezüglich der Nährstoffzufuhr in der postoperativen Phase und bei akuter Pankreatitis hat in den letzten Jahren ein Paradigmenwechsel stattgefunden: weg von der prolongierten Nahrungskarenz hin zu einer frühen enteralen Ernährung.
