Zusammenfassung
Der Ernährungsstatus ist Grundlage und Ausdruck der Gesundheit und charakterisiert den Zustand des Organismus in Hinblick auf die Körperzusammensetzung (Muskelmasse, Fett- bzw. fettfreie Masse) als auch der ausreichenden Verfügbarkeit von Mikronährstoffen und Spurenelementen (z. B. Vitamin B1, Folsäure oder Kalzium). Die Ermittlung des Ernährungsstatus hat auf der einen Seite das Ziel eine bestehende oder drohende Mangelernährung zu detektieren und auf der anderen Seite eine eingeleitete Ernährungstherapie zu überwachen und ggf. zu modifizieren. Neben der Detektion einer Fehlernährung im Sinne einer Unter- bzw. Mangelernährung hat auch die strukturierte Charakterisierung der überernährung bei stetig steigendem Anteil an adipösen Patienten einen wichtigen Stellenwert. Beide – Unter- und Mangelernährung und Adipositas – haben spezifische Implikationen nicht nur auf die Ernährungstherapie, sondern auch auf andere Therapiebereiche in der Intensivmedizin wie z. B. Beatmungsformen und Weaning oder Pharmakokinetik. Die idealen Marker zur Bestimmung des Ernährungsstatus sollten überall verfügbar, reproduzierbar, nicht aufwendig und kostengünstig sein, dabei möglichst spezifisch den Ernährungsstatus und dessen Schwankungen wiedergeben. Es sei hier schon vorweggenommen, dass es diese idealen Marker so nicht gibt. In der Intensivmedizin stellt die Ernährungstherapie in Form einer klinischen Ernährung eine wesentliche Säule im gesamten multimodalen Therapiekonzept dar. Die Erhebung des Ernährungszustandes unter Berücksichtigung des klinischen Krankheitsverlaufs und der bestehenden krankheitsspezifischen Probleme bildet dabei die Basis zur korrekten Indikationsstellung für die künstliche Ernährung und deren Therapieüberwachung im Verlauf.
