Zusammenfassung
Im Kapitel Pathophysiologie werden die wichtigsten ernährungsrelevanten Organstrukturen und deren vorrangige Störungen bei Intensivpatienten erläutert. Zunächst erfolgt eine funktionelle Beschreibung von ösophagus und Magen sowie der Magen-Darm- Motilität. Die pharmakologische Beeinflussung der Motilität und besondere Merkmale der postoperativen Motilität werden ausführlich behandelt. Obstipation und intestinale Translokation gehören zu den häufigsten pathophysiologischen Störungen der Intensivpatienten, dennoch erscheinen die therapeutischen Optionen limitiert. Stressblutungen nach peptischen Läsionen können dagegen durch sorgfältige Prophylaxe verhindert oder zumindest deutlich reduziert werden. Die gestörte Darmperfusion ist durch die Ernährung des Intensivpatienten beeinflussbar. Unklar sind hingegen die Ernährungsempfehlungen bei Schockleber, da die enterale Ernährung oft gestört und die parenterale Ernährung mit hepatischen und cholestatischen Nebenwirkungen assoziiert ist.
