Zusammenfassung
Die sogenannte Immunonutrition gehört derzeit zu den umstrittenen Therapieverfahren bei kritisch kranken Patienten. In den ESPEN-Leitlinien der Europäischen Gesellschaft für klinische Ernährung wird die Immunonutrition empfohlen (Empfehlungsgrad A). Allerdings sind die Ergebnisse wesentlich weniger klar bei Patienten, die sich vor ihrer akuten Erkrankung normal ernährt hatten und bei allen kritisch kranken Patienten. Bei diesen gibt es sogar Berichte über eine erhöhte Komplikationsrate oder auch eine erhöhte Sterblichkeit unter Immunonutrition bei septischen Patienten mit Pneumonien (Dent 2003). Ein Hauptgrund ist, dass es keine einheitliche Definition einer Immunonutrition gibt, sondern in den bisherigen Studien, die zum größten Teil erhebliche methodische Schwächen aufweisen, unterschiedliche Substrate in unterschiedlicher Dosierung miteinander kombiniert und klinisch getestet wurden. Ein individueller Effekt einzelner Substanzen ist zwar beschrieben, wenn jedoch die einzelnen Komponenten in Kombination gegeben werden, ist der Effekt der Einzelsubstanzen nicht vorhersehbar, da zu erwarten ist, dass Interaktionen zwischen den Substanzen vorhanden sind. Im Wesentlichen handelt es sich bei den verwendeten Kombinationen um Arginin/Glutamin, Omega- 3-Fettsäuren, Nukleotide, oder Antioxidantien (Vitamin E, Vitamin C, Zink, Selen). Allen diesen Substanzen werden immunmodulatorische Effekte in verschiedenen Systemen und Regulationskreisen zugeschrieben (◘ Tab. 13.1). Nur Studien zur Pharmakonutrition können einen spezifischen Effekt nachweisen, der dann auch dosisabhängig sein sollte. Erste Ansätze gibt es dazu für Selen. Im Folgenden sollen einige der Substanzen kurz besprochen werden.
