Zusammenfassung
Eine Krebserkrankung selbst sowie deren intensive medizinische Behandlung haben einen enormen, häufig lang anhaltenden Einfluss auf das körperliche, funktionale, emotionale und soziale Wohlbefinden des Krebspatienten (Courneya 2003). Die meist lange Behandlung führt oft zu einer reduzierten Mobilität , die eine Minderung der körperlichen Leistungsfähigkeit nach sich zieht. Wie diverse „Bed-rest“-Studien zeigen, sind u.a. Muskelatrophie (20–30% nach sieben Tagen), Abnahme der Knochendichte, Abnahme des Herzvolumens, Erhöhung der Ruhepulsfrequenz, erhöhtes Thrombose- und Pneumonierisiko sowie eine reduzierte Koordination und Sensomotorik Folgen der Immobilität (Bühring et al. 2011; Rittweger et al. 2010; Ferrando et al. 2002; Hollmann u. Hettinger 2000). Die psychische und psychosoziale Belastung sowie mögliche physische Nebenwirkungen der Behandlung tragen zusätzlich zur körperlichen Leistungsminderung bei (Newton u. Galvao 2008; Dimeo 2002). Kortikosteroide, ein beschleunigter Abbau von Muskelproteinen, pro-inflammatorische Zytokine sowie eine reduzierte Energieaufnahme führen neben der Immobilität zur Muskelatrophie (Al-Majid u. McCarthy 2001; Lucia et al. 2003; Oldervoll et al. 2004). Insbesondere die Atrophie der Hauptmuskelgruppen wie des Quadrizeps resultiert in einer verminderten Gleichgewichtskontrolle und führt somit zur Gangunsicherheit . Der Circulus vitiosus der Immobilität und Leistungsminderung ist beschritten. Die Inaktivität hat zur Folge, dass die körperliche Leistungsfähigkeit immer weiter abnimmt und auch Alltagsbelastungen erschwert durchzuführen sind (Humpel u. Iverson 2005).
