Zusammenfassung

Stenosen der Trachea, abgesehen von den seltenen angeborenen Formen aufgrund von Gefäß- oder Knorpelanomalien, entwickeln sich im Anschluss an ein die Trachea treffendes Trauma. Systematisch müssen Trachealstenosen nach der Ursache der Schädigung und nach der Lokalisation der Stenose unterschieden werden. Ursächlich kommen hierfür zwei verschiedene Schädigungsmechanismen infrage: erstens, das endoluminale Intubationstrauma zumeist infolge einer Langzeitbeatmung und zweitens, die extraluminale Schädigung nach einer Tracheotomie. Die Lokalisation der Stenose kann einerseits isoliert die zervikale Trachea treffen, andererseits höher liegen und den Larynx mit einbeziehen, so dass eine laryngotracheale Stenose resultiert. Diese Unterscheidung ist von eminenter Bedeutung, da die chirurgische Therapie einer laryngotrachealen Stenose ungleich schwieriger und komplikationsträchtiger ist, als die der isolierten Trachealstenose.
