Zusammenfassung

Ziele der operativen Behandlung von Läsionen der A. subclavia (AS) sind die Verbesserung der Durchblutungssituation des Armes, der A. thoracica interna (Koronarchirurgie) und die Ausschaltung von Aneurysmen sowie die Verbesserung der zerebralen Perfusion bei zentralen subklavialen Verschlussprozessen mit einem vertebrobasiliären Steal-Syndrom. Diese können durch eine transsubklaviale Thrombektomie (TE) oder Thrombendarteriektomie (TEA), eine Transposition der AS in die A. carotis communis, einen karotido-subklavialen Bypass oder einen subklavio-axillären Bypass erreicht werden. Operationen an der AS finden in einem chirurgisch anspruchsvollen Gebiet statt; an Komplikationen sind daher vor allem Blutungen (bis hin zur akuten Massenblutung) möglich. Selten sind zentrale neurologische Defizite, periphere Nervenschäden oder Lymphfisteln.
