Zusammenfassung
Die Femoralisgabel liegt im Trigonum femorale und wird von der A. femoralis communis (AFC) sowie ihrer Aufteilung in die A. profunda femoris (APF) und A. femoralis superficialis (AFS) gebildet. Ziele der Behandlung sind die Wiederherstellung der Durchblutung von Becken- und Beinarterien. Rekonstruktionsmethoden umfassen die Thrombendarteriektomie (TEA), die Thrombektomie (TE) von thromboembolischen Verschlüssen, die häufig die AFC betreffen und isolierte Bypass-Verfahren/Interponate. Femoralisgabelrekonstruktionen sind neben Eingriffen an den Becken- und Beinarterien erforderlich, um eine Gesamtdurchblutung sicherzustellen. Sonderformen sind Aneurysmen (Aneurysma verum bzw. spurium), Traumata und vereinzelt Tumoren. Durch die anatomische Nähe zu den Lymphgefäßen in der Leiste und zur bakterienlastigen Perinealregion sind lokale Infektionen in diesem Gebiet häufig. Die Arrosionsblutung ist die gefürchtetste Komplikation. Seltener sind Nachblutungen, periphere Thrombosen/Embolien, Lymphfisteln/-zysten und passagere Nervenalterationen. AV-Fisteln haben ihre Hauptursache in vorausgegangenen Interventionen.
