Zusammenfassung
Physiologischerweise ist die spontane Schrittmacherfunktion im Sinusknoten lokalisiert. Der Sinusknoten befindet sich im Bereich des Sulcus terminalis an der Mündungsstelle der V. cava superior in den rechten Vorhof. Die Blutversorgung erfolgt bei 55–60% der Patienten über die rechte Herzkranzarterie. Histologisch sind neben atrialen Muskelzellen und Transitionalzellen spezielle nodale Zellen (auch Pacemaker-Zellen genannt) im Sinusknoten nachweisbar. Diese »P-Zellen« stellen die eigentlichen Impulsgeber dar. Zum AV-Knoten erfolgt die Erregungsausbreitung über drei internodale Leitungsbahnen, zum linken Vorhof über das Bachmannsche Bündel.
