Zusammenfassung
Gentherapie ist bei monogenetischen Erkrankungen konzeptionell einfach: Ein mutiertes Gen, das zu zystischer Fibrose, Sichelzellenanämie, Adenosindesaminase-Mangel (mit schwerem kombiniertem Immundefekt), Muskeldystrophie etc. führt, wird durch ein funktionelles Gen ersetzt. Tatsächlich ist es aber nicht trivial das Genom des Menschen für eine sichere und effektive somatische Gentherapie zu manipulieren. Die ersten Gentherapieversuche am Menschen wurden bereits 1990 initiiert; 20 Jahre später ist die somatische Gentherapie nach wie vor im experimentellen Stadium. Die Keimbahntherapie ist in den meisten Ländern explizit verboten und nach dem heutigen Kenntnisstand noch nicht durchführbar. Kleinere Fragmente von Nukleinsäuren eignen sich aber für die Regulation der mRNA-Expression oder für die Bindung und Neutralisation von Proteinen.
