Zusammenfassung

Rund 6000 Kinder kommen jährlich in Deutschland mit angeborenem Herzfehler zur Welt. Dies entspricht einer Prävalenz von 1 % aller Neugeborenen, bei Frühgeborenen oder Mehrlingen ist sie jedoch doppelt so hoch [62, 108]. Ein Drittel der Kinder mit Herzfehler benötigt in der Neonatalperiode eine medikamentöse, katheterinterventionelle oder operative Behandlung [92]. Postnatal ist der Kreislauf erheblichen Umstellungsvorgängen unterworfen. Dadurch werden einige in utero gut tolerierte angeborene Herzfehler in den ersten Stunden nach der Geburt symptomatisch. Heute wird die Mehrzahl der komplexen Herzfehler pränatal diagnostiziert. Die Häufigkeit und Treffsicherheit der Pränataldiagnostik weist große regionale Unterschiede auf und ist von der Art des Herzfehlers abhängig. So wird z. B. die totale Lungenvenenfehlmündung oft nicht pränatal erkannt, da kaum Blut über die Lungenvenen fließt.
