In einem Kranken haus gibt es immer wieder Zwischenfälle, über die nicht gesprochen wird. Einmal wird ein Patient am falschen Knie operiert; obwohl eine Pflegekraft und ein Assistenzarzt Zweifel an der Richtigkeit der OP hatten, äußerten sie diese nicht. Auf Initiative eines Chefaiztes hin wird daraufhin ein Zwischenfallsberichtssystem eingeführt. Mitarbeiter des OP-Bereichs sollen Fehler und Probleme aller Art berichten. Dazu wird ein Formular entworfen, das im Aufenthaltsraum zusammen mit einer Erklärung ausliegt. Ein Vertrauensgremium wird gewählt, das die Berichte auswertet und mit den zuständigen Bereichsleitungen über Verbesserungen sprechen soll.
