Zusammenfassung
Fehlstellungen des proximalen Femurs sind auf fehlverheilte Frakturen und (oder inadäquate primäre Repositionen) zurückzuführen. Sie führen auch zu Dysbalancen der glutealen Muskulatur und in der Konsequenz zu Schäden im Bereich des gesamten Bewegungsapparats, vor allem aber in der Lendenwirbelsäule und den Kniegelenken. Somit ist die Klinik durch zunehmende Schmerzen und arthrotische Veränderungen geprägt. Die Grundprinzipien in der Behandlung der Fehlstellungen richten sich immer nach deren Ursache. Grundsätzlich sind vier posttraumatische Folgezustände denkbar, die durch eine korrigierende Osteotomie behandelt werden können. Diese sind: rezidivierende Hüftluxationen oder -subluxationen, Femurkopfnekrosen, Pseudarthrosen und Deformitäten. Die varische Fehlstellung nach pertrochantären Femurfrakturen und Implantatversagen stellt die häufigste sekundäre Fehlstellung dar. Mit der Korrektur der Fehlstellungen werden folgende Ziele verfolgt: Verbesserung der Funktion, Druckentlastung, Verbesserung der Krafteinleitung und die weitgehende Wiederherstellung der originären Anatomie.
