Zusammenfassung
Silber hat seit vielen tausend Jahren seinen festen Platz in der antimikrobiellen Behandlung. Kurzfristig wurde es durch die Entwicklung der Antibiotika im letzten Jahrtausend in den Hintergrund gedrängt. Jedoch hat es schnell aufgrund der sich rasant entwickelnden Antibiotikaresistenz wieder seinen berechtigten Platz in der Wundbehandlung gefunden. Dabei gibt es verschiedene Technologien, die in unterschiedlichen Verbandsstoffen zum Einsatz kommen. Deren Kenntnis ist essentiell, um diese in einem adäquaten Nutzen-Risiko-Verhältnis einsetzen zu können. Dabei weichen in vitro-Daten wie oft in der Medizin von in vivo-Untersuchungen ab, da Labormodelle nicht 1:1 in die Praxis umzusetzen sind.
Entsprechend dem Positionspapier der Europäischen Wund Management Gesellschaft (EWMA) gelten folgende Schlüsselpunkte:

Silber ist ein Breitspektrum-Agenz mit einer geringen Toxizität in der Wundapplikation.

2.
Silber ist aktiv in der ionisierten Form, die Konzentration ist abhängig von der Silbersalzlöslichkeit.

3.
Bakterienresistenzen wurden beschrieben.

4.
Die Verwendung von Silber in Wundverbänden muss durch weitere wissenschaftliche und klinische Studien evaluiert werden.
