Zusammenfassung
Für die operative Behandlung von Hernien werden seit mehr als 40 Jahren alloplastische Materialien, sog. Meshes, erfolgreich eingesetzt (Usher 1959). Nachdem eine deutliche Senkung der Rezidivrate durch Verwendung von Meshes in der Chirurgie der Narbenhernie gezeigt werden konnte, erfuhr der Einsatz alloplastischer Materialien eine zunehmende Verbreitung und die Indikationen wurden ausgeweitet. Neue offene Verfahren zur Leistenhernienreparation mit Verstärkung durch Meshes (OP nach Lichtenstein) als auch seit Beginn der 90er Jahre zunehmend laparaskopische Techniken mit Implantation von alloplastischem Material (TEP, TAPP) haben mit dazu beigetragen, dass in der Bundesrepublik heutzutage schätzungsweise mehr als 50% aller Hernien mit Meshes versorgt werden und weltweit ca. 1 Mio. Netze jährlich implantiert werden. Die mittlerweile breite Akzeptanz und Etablierung der Verwendung alloplastischer Materialien sollte jedoch nicht darüber hinwegtäuschen, dass spezifische Fremdkörperreaktionen des Körpers auf das Material zu meshbezogenen Komplikationen führen können. Darüber hi naus gibt es Hinweise in der Literatur, dass die Wahl des verwendeten Materials einen Einfluss auf das Ausmaß der Gewebereaktion und somit auf potenziellen Komplikationen hat (Amid 1997, Leber et al. 1998; ▶ Kap. 5, ▶ Kap. 6), was durch tierexperimentelle Studien untermauert werden konnte (Bellon et al. 1997, Klinge et al. 1998, Klosterhalfen et al. 1998). Die Beurteilung der absoluten Inzidenz von meshbezogenen Komplikationen ist durch das Fehlen kontrollierter Studien bezüglich des Langzeitverlaufs der Fremdkörperreaktion und klinischer Ergebnisse erschwert.
