Zusammenfassung
Neuroendokrine GEP-Tumoren sind eine heterogene Gruppe seltener Neoplasien, die in 2 Gruppen unterteilt werden: endokrine Pankreastumoren und endokrine Tumoren des Magen-Darmt-Takts (Karzinoide). Gemeinsam ist ihnen die Synthese und Sekretion einer Vielzahl biogener Amine und Peptidhormone. Nach dem klinischen Bild wird zwischen funktionell aktiven und funktionell nichtaktiven Tumoren unterschieden, wobei funktionell aktive Tumoren entsprechend der hormonellen Wirkung definierte klinische Syndrome verursachen, während sich die nichtfunktionell aktiven Tumoren entweder als Zufallsbefund oder aufgrund raumfordernder Wirkung manifestieren.
Oberstes Therapieprinzip ist die chirurgische Resektion. Bei metastasierten Tumoren steht die symptomatische Behandlung im Vordergrund. Erst bei nachgewiesenem Tumorprogress oder bei kleinzelligen Karzinomen sind antiproliferative Behandlungsstrategien indiziert.
