Wirksamkeit der „Aktiven Gesundheitsförderung im Alter“

Zielgruppenspezifische Ergebnisse bezüglich einer Kompression von Morbidität über 13,8 Jahre LUCAS Verlauf

Zusammenfassung

Hintergrund und Ziel der Arbeit

Die Weltgesundheitsorganisation (WHO) definiert gesundes Altern als Entwicklung und Erhalt funktionaler Kompetenz. Eine ungünstige Form des Alterns wird mit „frailty“ (Gebrechlichkeit) beschrieben, einem sukzessiven Abbau von Funktionen. Dieser Frailty-Prozess ist positiv beeinflussbar. Bisherige Interventionen galten meist Personen im Krankenhaus oder im Pflegeheim (stationären Setting). In dieser Arbeit wird der Erhalt funktionaler Kompetenz im kommunalen Setting untersucht.

Material und Methoden

Das Programm „Aktive Gesundheitsförderung im Alter“ wurde für selbstständige Menschen ab 60 Jahre und älter und ohne Behinderung von einem Gesundheitsberater-Team mit geriatrischer Expertise durchgeführt und in der Longitudinalen Urbanen Cohorten-Alters-Studie (LUCAS) über 13,8 Jahre auf Wirksamkeit überprüft. Lebenszeit und behinderungsfreie Lebenszeit wurden separat für funktional kompetente Personen (viele Reserven) bzw. Personen mit wenigen funktionalen Reserven mittels Kaplan-Meier Kurven verglichen. Für mögliche Ungleichverteilungen bezüglich Alter, Geschlecht, Bildung, chronische Krankheiten und Funktionsstatus wurde mit multivariaten Cox Regressionen adjustiert. Dies ermöglicht verlässliche Aussagen zu Zusammenhängen zwischen Mortalität und Morbidität (Kompression von Morbidität), einschließlich Einflüssen durch Lebensstilinterventionen.

Ergebnisse

Funktional kompetente Teilnehmende lebten signifikant länger ohne Behinderung (p < 0,001). Zudem war der durchschnittliche Anteil an Lebenszeit mit Behinderung für sie deutlich kleiner als für alle anderen Gruppen (funktional wenig Kompetente und Nichtteilnehmende).

Diskussion

Die Analysen zur Kompression von Morbidität zeigten, dass dieses Programm am stärksten bei funktional kompetenten älteren Menschen wirkte, für die es entwickelt worden war.
