Todesfälle durch scharfe Gewalt in Berlin – notärztliche und forensische Aspekte

Zusammenfassung

Einleitung

Todesfällen durch scharfe Gewalt muss in Deutschland gerade auch angesichts der restriktiven Schusswaffengesetze eine hohe Bedeutung beigemessen werden. Bundesweite Erfassungen hierzu fehlen jedoch. In einer retrospektiven Fallanalyse untersuchten wir die Umstände von Todesfällen durch scharfe Gewalt im Zeitraum 2005–2015 in Berlin.

Material und Methoden

Es erfolgte eine retrospektive Auswertung der 10.514 in diesem Zeitraum am Institut für Rechtsmedizin der Charité – Universitätsmedizin Berlin durchgeführten gerichtlichen Leichenöffnungen.

Ergebnisse

Bei 191 Todesfällen durch scharfe Gewalt (1,8 % aller Sektionen) handelte es sich in 52,9 % um Suizide und in 40,8 % um Tötungsdelikte. Sowohl bei den Suiziden als auch bei den Homiziden stellte das Verbluten mehrheitlich die Todesursache dar, wobei ein vermehrtes Auftreten der Todesursachen „Hämato‑/Pneumothorax“ und „Blutaspiration“ bei den Homiziden zu verzeichnen war. Während bei Suizidenten die tödliche(n) Verletzung(en) mehrheitlich im Bereich der oberen Extremitäten zu finden war(en), waren Opfer homizidaler Gewalt mehrheitlich in der Brustregion tödlich verletzt.

Schlussfolgerung

Die Relevanz von Todesfällen durch scharfe Gewalt und notfallmedizinische sowie forensische Spezifika werden herausgearbeitet. Insbesondere wird die Bedeutung der Blutungskontrolle sowie die Relevanz der Minithorakotomie bei traumatischem Kreislaufstillstand mit penetrierender Thoraxverletzung deutlich. Begünstigt durch die gute Verfügbarkeit entsprechender Tatwaffen werden Gewalttaten mittels scharfer Gewalt auch zukünftig eine wichtige Rolle unter kriminalistischem und notfallmedizinischem Aspekt spielen.
