Zusammenfassung

Verschiebungen des Größenverhältnisses zwischen Herz und Lungen sowie die Verlagerung von Herz und Mediastinum sind häufig Ausdruck einer intrathorakalen Pathologie oder eines Zwerchfelldefekts. Zu den häufigsten Lungenfehlbildungen gehören die kongenitale pulmonale Atemwegs-Malformation (congenital pulmonary airway malformation, CPAM), vaskuläre Anomalien und kombinierte Anomalien von Lunge und Gefäßsystem (z. B. Sequestration). Thorakale Anomalien können zu einer Lungenhypoplasie und damit zu einer erhöhten postnatalen Morbidität und Mortalität führen. Die die häufigsten intrathorakelen Ursachen der Lungenhypoplasie sind die fetale Zwerchfellhernie, die CPAM oder andere congenitale cystische pulmonale Läsionen (congenital cystic pulmonary lesions, CCPL). Die Entbindung von Kindern mit diesen Krankheitsbildern sollte immer am Perinatalzentrum erfolgen, da viele der Neugeborenen symptomatisch sind und eine entsprechende neonatologische Intensivtherapie bis hin zur sofortigen kinderchirurgischen Intervention benötigen.
