Zusammenfassung

Schwere Tauchunfälle sind potentiell lebensbedrohliche Ereignisse, die durch einen raschen Abfall des Umgebungsdrucks beim Auftauchen hervorgerufen werden und mit der Bildung von freien Gasblasen im Blut und Gewebe einhergehen. Unabhängig davon, ob eine Dekompressionskrankheit (DCS) oder eine arterielle Gasembolie (AGE) vorliegt, sind die wichtigsten Sofortmaßnahmen die schnellstmögliche Gabe von Sauerstoff in höchstmöglicher Konzentration und die Volumentherapie. Wichtigste weiterführende Maßnahme ist die schnellstmögliche Rekompression in einer Druckkammer mit hyperbarem Sauerstoff.
Ertrinkungsunfälle stellen international v. a. bei jüngeren Menschen in den Todesursachenstatistiken einen erheblichen Anteil dar. Das wesentliche Problem beim Ertrinken ist ein Verlust von Gasaustauschfläche mit nachfolgender Hypoxie. Wichtig sind daher eine schnelle Rettung und eine rasche Hypoxiebehandlung. Häufig besteht zudem eine begleitende Hypothermie, was zum einen per se einen lebensbedrohlichen Zustand darstellt, zum anderen aber unter günstigsten Bedingungen die Chance auf eine erfolgreiche Reanimation verbessern kann.
