Serumhepcidin bei Eisenmangelanämie und Anämie chronischer Erkrankungen im geriatrischen Kollektiv

Hintergrund

Die Eisenmangelanämie und Anämie chronischer Erkrankungen (ACE) sind im höheren Lebensalter am häufigsten. Eine Abgrenzung beider Formen wird durch das Fehlen einer einheitlichen Definition der ACE erschwert. Hepcidin als Eisenregulator begünstigt durch Herabregulation bei Eisenmangel eine enterale Eisenresorption und beeinträchtigt sie durch Hochregulation bei z. B. chronischer Entzündung. Ziel unserer Studie war die Untersuchung, ob Hepcidin als diagnostischer Parameter zur Differenzierung zwischen Eisenmangelanämie und ACE herangezogen werden kann.

Patienten und Methoden

Insgesamt 37 stationäre Patienten (69–97 Jahre) wurden in 4 Gruppen eingeteilt. Gruppe I bildeten Patienten mit Eisenmangelanämie, Gruppe II Patienten mit ACE, Gruppe III war die Kontrollgruppe und Gruppe IV waren Patienten mit Eisenmangelanämie/ACE. Hepcidin wurde mithilfe eines kommerziell erwerbbaren Hepcidin-ELISA-Tests bestimmt (DRG Instruments, Marburg). Die Hepcidinspiegel wurden mit nichtparametrischen Methoden verglichen.

Ergebnisse

Die Unterschiede im Hepcidinspiegel waren signifikant, sowohl im Vergleich aller 4 Gruppen (p = 0,034) als auch im bivariaten Vergleich zwischen Gruppe II und III (p = 0,003). Hepcidin und Ferritin waren positiv korreliert mit einem Spearman-Rho-Koeffizienten von 0,747.

Schlussfolgerung

Trotz der kleinen Fallzahl und damit nur eingeschränkten Aussagekraft der vorliegenden Studie weisen die Ergebnisse in die gleiche Richtung wie aktuelle Literaturquellen: Hepcidin kann sich in Zukunft zu einem möglichen Diagnostikkriterium zur Differenzierung zwischen Eisenmangelanämie und ACE entwickeln. Weiterführende, randomisierte Studien an größeren Kollektiven sind jedoch zur endgültigen Klärung dieser Frage dringend erforderlich.
