Stellenwert der Multidetektor-Computertomographie (MDCT) in der frühen Versorgung von Traumapatienten

Zusammenfassung

Unfälle verursachen eine höhere Mortalität und einen höheren Verlust an Lebensjahren als Tumor- und kardiovaskuläre Erkrankungen zusammen. In der Altersgruppe bis 45 Jahre ist das Polytrauma sogar die häufigste Todesursache in industrialisierten Ländern. Ein wesentlicher Faktor für die Prognose des unfallverletzten Patienten ist der Faktor Zeit, genauer gesagt die Zeit, die bis zur Einleitung einer kausalen Therapie verstreicht. Diese Tatsache nimmt sowohl die präklinisch tätigen Rettungskräfte als auch das Team in die Pflicht, welches sich in der frühen Phase nach einem Unfall der Versorgung der Patienten annimmt.
Die Radiologie ist als Mitglied des im Schockraum tätigen Trauma-Teams vor allem dafür verantwortlich, dass verletzte Patienten rasch und, an den Prioritäten orientiert, mit adäquaten Mitteln einer bildgebenden Diagnostik unterzogen werden.
Basierend auf wissenschaftlicher Evidenz ist das am besten geeignete diagnostische Mittel die Computertomographie (CT). Mit dem Einsatz der modernen Multidetektor-Computertomographie (MDCT) ist es dem Radiologen möglich, innerhalb weniger Sekunden über lebensbedrohliche Verletzungen Auskunft zu geben. Die konventionelle Röntgendiagnostik (CR) konnte durch die MDCT nahezu vollständig ersetzt werden. Die Indikationen zur MDCT weiten sich zunehmend aus, auch metastabile Patienten werden mit MDCT untersucht. Neuere Untersuchungen konnten zeigen, dass das Überleben positiv mit der Durchführung einer Ganzkörper-CT korreliert.
Im vorliegenden Artikel werden die Indikationen zur CT des Thorax und des Abdomen in der frühen Versorgung traumatisierter Patienten und die häufigsten Verletzungen beleuchtet.
