Risikofaktoren für Outcome und Mortalität bei hospitalisierten geriatrischen Patienten mit SARS-CoV-2-Infektion

Daten aus einem Maximalversorger im Zeitraum der zweiten Coronawelle 2020/2021 in Deutschland

Zusammenfassung

Hintergrund

Im Winterhalbjahr 2020/2021 durchlebte Deutschland die zweite Coronainfektionswelle. Wie bereits zuvor in den ersten beiden Wellen wurden v. a. Bewohner von Alten- und Pflegeheimen mangels eines verfügbaren Impfstoffes besonders hart getroffen. Die Mortalität in dieser Altersgruppe war aufgrund der häufig vorhandenen Multimorbidität hoch.

Methodik

Untersucht wurden 168 geriatrische Patienten mit nachgewiesener SARS-CoV-2-Infektion, die im Zeitraum vom 01.10.2020 bis zum 31.03.2021 stationär behandelt wurden. Es erfolgten die retrospektive Auswertung der Patientenakten und eine deskriptive Auswertung von Symptom- und Krankheitsdauer, Laborparametern und Begleiterkrankungen. Ziel war es, Risikofaktoren für einen tödlichen Verlauf herauszuarbeiten.

Ergebnisse

Eingeschlossen wurden 168 Patienten im Alter von 65 bis 97 Jahren. Die durchschnittliche Mortalität lag bei 28 % und war in der Altersgruppe über 90 Jahren am höchsten. Die meisten Patienten verstarben innerhalb der ersten 10 Tage. Eine Intensivtherapie verlängerte zwar den stationären Aufenthalt um gut 6 Tage, das Überleben glich sich bis zum Ende der Beobachtung aber dem von nichtintensivmedizinisch Behandelten an. Risikofaktoren für eine Intensivbehandlung waren eine Neutrophilie, Lymphopenie sowie hohe Werte für Ferritin und D‑Dimere. Das Risiko eines tödlichen Ausgangs wurde durch ein hohes Alter, eine kurze Symptomdauer, eine vorbekannte Demenz sowie die Einnahme von Neuroleptika und Antidepressiva bestimmt.
