Die LUCAS-I Marker zur Klassifizierung älterer Menschen als FIT, pre-FRAIL und FRAIL. Validierung und erste Ergebnisse aus der Longitudinalen Urbanen Cohorten-Alters-Studie (LUCAS)

Zusammenfassung

Hintergrund

Es gibt Bedarf für ein einfaches Selbstausfüller-Instrument zur Erkennung von Frailty in der heterogenen, selbstständig lebenden älteren Bevölkerung.

Methoden

Vorgestellt wird ein neuer Fragenkatalog zur Einschätzung des funktionellen Status älterer Menschen. Im Unterschied zur Literatur wird der Fokus gleichermaßen auf Risiken und Ressourcen gelegt. Die Fragen beziehen sich eher auf Fakten (Intensität, Dinge zu tun) als auf subjektive Einschätzungen. Der Fragenkatalog wurde im LUCAS-Projekt entwickelt.

Ergebnisse

Die auf diesem Markerset basierende Klassifizierung erlaubt die Einteilung in die Gruppen FIT, pre-FRAIL sowie FRAIL und ist prädiktiv für die Entwicklung von Pflegebedürftigkeit und Tod. Eine Fülle von Resultaten erbringt verglichen mit anderen Gesundheitsparametern den Nachweis für die Validität dieser Klassifizierung. Eine Frage betrifft „Fahrradfahren“. Wo dies nicht angemessen ist, wird der Ersatz durch „500 m Gehen ohne Hilfe“ empfohlen. „Radfahren“ ist jedoch sensitiver.

Schlussfolgerung

Der Fragebogen ermöglicht einen einfachen und kosteneffektiven Weg, ältere Menschen frühzeitig auf Anzeichen von Funktionsverlusten für Präventionsmaßnahmen zu screenen.
