Für Anästhesistinnen und Anästhesisten sind präoperative Risikoevaluation, intraoperativ engmaschiges Monitoring und postoperativ adäquate Patientenversorgung selbstverständlich; letzteres gilt sowohl im Aufwachraum als auch unter intensivmedizinischen Bedingungen. In den letzten Jahren erhalten wir zunehmend Anhaltspunkte, dass unmittelbar postoperativ und im weiteren Verlauf insbesondere ältere Patienten an postoperativem Delir und kognitiven Störungen leiden. Die Frage, inwieweit wir Störungen als Voraussetzung für eine frühe Therapie überhaupt detektieren – und welche Rolle dabei unser anästhesiologisches und intensivmedizinisches Vorgehen einnimmt –, ist Gegenstand dieses Topthemas.
