Die antithrombotische Therapie ist eine wichtige Komponente der Behandlung von Patienten mit akutem Koronarsyndrom (ACS). Während die Antikoagulation meist nur in der Akutphase benötigt wird, kommt dem Einsatz antithrombozytärer Substanzen eine große Bedeutung in der Akut-, Erhaltungs- und Langzeittherapie des ACS zu. Aktuell ist eine duale antithrombozytäre Therapie (DAPT) mit ASS und einem potenten P2Y12-Inhibitor für 12 Monate Standardtherapie. Da das insbesondere in der initialen Phase nach ACS erhöhte ischämische Risiko im Verlauf abnimmt, das Blutungsrisiko aber kumulativ erhöht bleibt, wurden DAPT-verkürzende und DAPT-deeskalierende antithrombozytäre Therapiekonzepte bei ACS-Patienten in einer Vielzahl von Studien und Metaanalysen untersucht. Die aktuelle ESC-Leitlinie zum akuten Koronarsyndrom von 2023 hat die neuen Therapiekonzepte und umfangreichen Studiendaten zur antithrombotischen Therapie nach ACS aufgenommen und gewertet und gibt Therapieempfehlungen im Sinne von Standard- und alternativen Strategien. Therapieentscheidungen zur antithrombotischen Therapie nach ACS sollten individuell in Abhängigkeit des ischämischen Risikos und Blutungsrisikos getroffen und ggf. im zeitlichen Verlauf angepasst werden.
Abstract
Antithrombotic therapy is an important component of the treatment of patients with acute coronary syndromes (ACS). While anticoagulation is usually only needed in the acute phase, the use of antiplatelet drugs is of great importance in the acute, maintenance and long-term treatment of ACS. Currently, dual antiplatelet therapy (DAPT) with ASA and a potent P2Y12 inhibitor for 12 months is standard therapy. The increased ischemic risk, especially in the initial phase after ACS, decreases over time, whereas the cumulative bleeding risk remains elevated. Therefore, abbreviated DAPT and DAPT de-escalation strategies in ACS patients have been examined in a large number of studies and meta-analyses. The current 2023 ESC guidelines for the management of acute coronary syndromes have incorporated and evaluated the new therapeutic concepts and extensive study data on antithrombotic therapy after ACS and provide treatment recommendations with default and alternative strategies. Treatment decisions on antithrombotic therapy after ACS should be made individually depending on the patients’ ischemic and bleeding risk and, if necessary, be adjusted over time.

Bei Diagnosestellung des ACS wird eine Vorbehandlung mit Acetylsalicylsäure (ASS) immer empfohlen, eine Routinevorbehandlung mit einem P2Y12-Inhibitor bei Patienten mit ST-Strecken-Hebungsinfarkt (STEMI) jedoch nur noch eingeschränkt, bei Patienten mit Nicht-ST-Strecken-Hebungs-ACS hingegen nicht mehr.

Eine duale antithrombozytäre Therapie (DAPT) mit ASS und Ticagrelor oder Prasugrel für 12 Monate bleibt Standardtherapie (Prasugrel bevorzugt); bei alten Patienten kann eine DAPT mit Clopidogrel erwogen werden.

DAPT-Verkürzung und DAPT-Deeskalation können als alternative Strategien bei perkutaner Koronarintervention (PCI) zur Senkung des Blutungsrisikos, insbesondere bei Patienten mit hohem Blutungsrisiko, eingesetzt werden.

Die Empfehlungen zur antithrombotischen Therapie für Patienten mit ACS und Indikation zur oralen Antikoagulation haben sich nicht geändert.

Eine Monotherapie mit ASS bleibt Standardtherapie in der Langzeitbehandlung > 12 Monate nach ACS; eine P2Y12-Inhibitor-Monotherapie kann erwogen werden, ebenso eine duale antithrombotische Therapie oder verlängerte DAPT bei erhöhtem ischämischem Risiko.

Therapieentscheidungen zur antithrombotischen Therapie nach ACS sollten individuell in Abhängigkeit des ischämischen Risikos und Blutungsrisikos getroffen werden.
