Hintergrund

Das Angebot an Trainingsprogrammen zur Optimierung der prähospitalen Versorgung Schwerverletzter in Deutschland ist vielfältig. Umstritten sind insbesondere die US-amerikanischen Programme International Trauma Life Support® (ITLS®) und Pre-Hospital Trauma Life Support® (PHTLS®), da sie ohne wesentliche Anpassungen auf das deutsche Rettungsdienstsystem angeboten werden. Mit dieser Übersichtsarbeit wurden in Deutschland verfügbare Programme systematisch identifiziert und deren Evidenz zur Wirksamkeit und Effektivität kritisch bewertet.

Material und Methoden

Systematische Recherche (Cochrane-Reviews und Clinical Trials, Embase, Pubmed, PsycInfo und CINAHL; 10/2010), Identifizierung von Trainingsprogrammen und kritische Bewertung aller Entwicklungs- und Evaluationsstudien.

Ergebnisse

2519 Titel wurden identifiziert, 18 Publikationen zu 4 Trainingsprogrammen (ITLS, PHTLS, Rummelsberg-Konzept und TraumaManagement®) wurden eingeschlossen. Für PHTLS wurden 3 Wirksamkeitsstudien identifiziert. Für kein Programm wurden Arbeiten zur Effektivität im deutschen oder ähnlichem Versorgungskontext identifiziert.

Schlussfolgerung

Eine Implementierung der in Deutschland verfügbaren Trainingsprogramme ist wegen mangelnder Evidenzbasierung schwer zu rechtfertigen. Es besteht Entwicklungs- und Forschungsbedarf.
