Präklinische Gerinnungsnotfälle bei Kindern

Zusammenfassung

Gerinnungsstörungen bei Kindern können in angeborene und erworbene Störungen der primären und sekundären Hämostase unterteilt werden. Ab dem 6. Lebensmonat ist die physiologische Entwicklung der Gerinnung etwa mit dem Hämostasepotenzial des Erwachsenen vergleichbar. Rettungsdienstlich relevante Störungen der Gerinnung umfassen dabei das von-Willebrand-Syndrom, die Hämophilie, aber auch die postoperative Nachblutung nach HNO-ärztlichen Eingriffen oder die Medikamenten-induzierten Störungen. Neben den bekannten gerinnungsbeeinflussenden Medikamenten wie Acetylsalicylsäure (ASS) oder Cumarinen können auch eine Reihe weiterer Medikamente, wie beispielsweise nichtsteroidale Antirheumatika und Valproat, erheblichen Einfluss v. a. auf die primäre Hämostase haben. Präklinisch ist eine genauere Differenzialdiagnose kaum möglich. Wichtig ist eher das Bewusstmachen für mögliche begleitende Störungen der Blutgerinnung, die ein Blutungsproblem ausgelöst oder zur Verstärkung der Blutung geführt haben können. Beim Verdacht auf eine zugrunde liegende angeborene oder erworbene Gerinnungsstörung sollte eine ausführliche persönliche und Familienanamnese einschließlich einer Medikamentenanamnese durchgeführt und das Kind in ein geeignetes pädiatrisches Zentrum eingewiesen werden. Bei bereits erfolgter Diagnose einer Gerinnungsstörung sollte ein Notfallausweis und ggf. auch Notfallmedikamente vorhanden sein. Wichtig ist das Erkennen auch solcher Kinder, die eine vermehrte Hämatomneigung und unerklärbare Verletzungen durch Kindesmissbrauch erlitten haben. Akute Blutungen werden wie beim Erwachsenen durch Kompression oder Ruhigstellung versorgt. Die Gabe von Tranexamsäure sollte insbesondere bei Schleimhautblutungen erwogen werden.
