19. Prädiktiver Ultraschall, Screening und Vorhersage für Präeklampsie und fetale Wachstumsrestriktion im II. und III. Trimester

Zusammenfassung

Die Dopplersonografie der Aa. uterinae im II. Trimester kann, mit besseren Detektionsraten als im I. Trimester grundsätzlich Fälle mit Präeklampsie und fetaler Wachstumsrestriktion (fetal growth restriction, FGR; auch intrauterine growth restriction, IUGR, genannt) vorhersagen, ist jedoch aufgrund ihrer geringen Sensitivität nicht als alleinige Screeningmethode geeignet. Eine multiparametrsiche Risikostratifizierung basierend auf maternalen Risikofaktoren und Charakteristika, zusätzlichen biochemischen Serummarkern und Dopplersonographie hat das Potenzial, Schwangere mit einem erhöhten Risiko und behandelbaren Risikofaktoren für Präeklampsie und auch FGR zu identifizieren, um sie frühzeitig einer personalisierten Therapie und intensiveren Überwachung der Schwangerschaft zuzuführen. Auch im III. Trimester kann durch dieses Vorgehen eine weitere Risikostratifizierung bezüglich später Präeklampsie, FGR und adversem neonatalen Outcome erfolgen und das perinatale Management entsprechend modifiziert werden.
