Zusammenfassung

Hintergrund

Der plötzliche Herztod (PHT) ist die häufigste Todesursache außerhalb von Krankenhäusern in modernen Industriegesellschaften. In 80 % der Fälle liegt dem PHT eine koronare Herzkrankheit zugrunde; die übrigen 20 % sind im Wesentlichen Folge eines weiten Spektrums angeborener bzw. erworbener Erkrankungen des Herzens.

Methode

Analyse klinischer Studien.

Ergebnisse

Bei genauerer Analyse der Umstände des PHT lassen sich eine Vielzahl typischer Gemeinsamkeiten feststellen: Über zwei Drittel aller PHT-Fälle ereignen sich in häuslicher Umgebung und wiederum zwei Drittel davon in Gegenwart von Augenzeugen (überwiegend Angehörige) und zum großen Teil nach typischen oft lang anhaltenden Prodromalbeschwerden, die den Symptomen bei koronarer Herzkrankheit sehr ähnlich sind. Patienten und Angehörige erkennen die Bedeutung dieser prodromalen Warnzeichen jedoch häufig nicht. Darüber hinaus sind die Laienreanimationsquoten in häuslicher Umgebung deutlich geringer als in der Öffentlichkeit – mit der Folge wesentlich schlechterer Überlebensraten.

Schlussfolgerung

Die Befunde weisen auf ein erhebliches Potenzial hin, den PHT zu bekämpfen: durch gezielte Information und Schulung von Patienten und Angehörigen zur Bedeutung der Warnsymptome, der Aufklärung über die richtigen frühen Reaktionen und der Notwendigkeit, die Basismaßnahmen der Wiederbelebung zu erlernen und im Notfall anzuwenden.
