Pharmakotherapie der Altersdepression

Zusammenfassung

Die antidepressive Pharmakotherapie des älteren Menschen ist weniger gut untersucht als die im jungen und mittleren Lebensalter, obwohl die Punktprävalenz für mittelschwere oder schwere Depressionen bei älteren Menschen etwa 4 % beträgt. Zudem hat sich gezeigt, dass Altersdepression die Mortalität erhöht. Bei der Therapie des älteren depressiven Menschen sind folgende Aspekte zu beachten: Komorbidität und als Folge die Polypharmazie, mögliche Interaktionen, physische Alterungsprozesse und ihr Einfluss auf die Pharmakodynamik und Pharmakokinetik der Medikamente sowie die individuelle psychosoziale Situation. Zwar haben ältere Menschen ein hohes Risiko, Nebenwirkungen unter einer antidepressiven Therapie zu entwickeln, aber die Behandlung ist wirksam und eine Phasenprophylaxe erfolgreich. Serotonin-Wiederaufnahmehemmer (SSRI) und kombinierte Serotonin- und Noradrenalin-Wiederaufnahmehemmer (SNRI) sind Mittel der ersten Wahl für die Akutbehandlung und Erhaltungstherapie der Depression im Alter. Mindestens eine 6-wöchige Therapie ist notwendig, um die Wirksamkeit zu überprüfen. Eine Augmentation mit Lithium kann bei therapierefraktärer Depression wirksam sein. Die Erhaltungstherapie sollte mindestens 9 Monate betragen und die Rezidivprophylaxe mindestens 2 Jahre.
