Pflegerische Betreuungskonzepte bei Patienten mit Demenz

Ein systematischer Review

Zusammenfassung

Einleitung

In Deutschland leiden derzeit etwa eine Million Menschen an einer Demenzerkrankung. Da diese in der Regel nicht heilbar sind, kommt der Pflege der Betroffenen eine besonders wichtige Rolle zu. Ziel der Betreuung ist eine möglichst lange Aufrechterhaltung kognitiver Fähigkeiten, Selbstständigkeit und Lebensqualität. Wichtige Ansätze hierzu sind die multisensorische Stimulation, Validation, Realitätsorientierung und Reminiszenz. Die Effektivität dieser Verfahren ist allerdings unklar.

Methoden

Es wurde ein systematischer Review durchgeführt. Eingeschlossen wurden randomisiert-kontrollierte Studien mit mindestens 30 Teilnehmern zur Effektivität von multisensorischer Stimulation, Validation, Realitätsorientierung oder Reminiszenz bei Patienten mit Demenz. Es gab keine Einschränkungen hinsichtlich der untersuchten Endpunkte der Studien.

Ergebnisse

Vierzehn Studien mit insgesamt 1513 Patienten erfüllen die Einschlusskriterien. Davon untersuchen 5 Studien die Reminiszenztherapie, 4 die multisensorische Stimulation, 3 die Validation und 2 die Realitätsorientierung. Die Studien erfassen Zielkriterien aus den Bereichen kognitive Fähigkeiten, Verhalten, psychische Belastung und Lebensqualität. Teilweise positive Ergebnisse werden hinsichtlich kognitiver Leistungen für Reminiszenz und Realitätsorientierung berichtet, hinsichtlich der anderen Zielkriterien sind die Ergebnisse überwiegend negativ.

Schlussfolgerung

Basierend auf der aktuellen Studienlage ist die Evidenz für die Effektivität der berücksichtigten Verfahren in der Betreuung von Patienten mit Demenz unzureichend. Es konnten allerdings nur wenige methodisch adäquate Studien identifiziert werden. Es sind daher weitere Interventionsstudien notwendig.
