Patientenseitiger Gesprächsbedarf nach einem geriatrischen Assessment in Hausarztpraxen

Zusammenfassung

Hintergrund

Der Gesprächsbedarf (GB) hausärztlicher Patienten wird anlassorientiert erhoben und bearbeitet. Ein geriatrisches Assessment zielt auf eine ganzheitliche Erfassung des Gesundheits- und Funktionszustands und ermöglicht eine Thematisierung von Problemen, die über aktuelle Beratungsanlässe hinausgehen.

Ziel der Arbeit

Ziel ist es, Ausmaß und Art des patientenseitigen GB nach einem hausärztlichen Assessment zu beschreiben und Einflussfaktoren auf den GB zu ermitteln.

Material und Methoden

Es erhielten 317 Patienten (≥ 70 Jahre) in 40 Hausarztpraxen ein geriatrisches Assessment. Die erhobenen Probleme wurden in einer Ergebnisliste dokumentiert und den Patienten im Anschluss vorgelegt. Diese bewerteten jedes Problem hinsichtlich subjektiver Relevanz und GB. In die Auswertung gingen Häufigkeitsanalysen zur subjektiven Relevanz und zum GB der vorliegenden Gesundheitsprobleme ein. In einem gemischten Modell wurden Einflussfaktoren auf den patientenseitigen GB untersucht.

Ergebnisse

Die 317 Patienten wiesen im Median 11 Gesundheitsprobleme auf [Interquartilsabstand (IQA) 8–14]. Von den Patienten hatten 80 % einen GB – und diesen zu wenigen Problemen. Ein hoher GB fand sich für körperliche Beschwerden und den Impfschutz. Wenig GB bestand für ungesunde Lebensgewohnheiten, Stürze, Alltagsfunktionen und psychosoziale Probleme. Im gemischten Modell hatten sowohl die Beschaffenheit des Problems als auch die beigemessene Relevanz einen unabhängigen Einfluss auf den GB.

Diskussion

Das geriatrische Assessment generiert einen mäßigen patientenseitigen GB. Es gibt Hausärzten die Gelegenheit, auch auf Gesundheitsprobleme zu achten, die von Patienten eher nicht thematisiert werden, obwohl sie ihnen wichtig sind. Dazu zählen Probleme in der Alltagsfunktion und im psychosozialen Bereich.
