6. Patientenauswahl für die laparoendoskopische Leistenhernienreparation

Zusammenfassung

Dieses Kapitel reflektiert die amerikanische Sicht der Patientenselektion für die laparoendoskopische Leistenhernienreparation. Es werden vor allem die Einschränkungen der lndikation zu minimalinvasiven Lösungen aufgezeigt, welche die üblichen Komorbiditäten der Hernienpatienten zum Ausdruck bringen. Bei der Wahl der Operationstechnik sollen viele Faktoren berücksichtigt werden: patientenbezogene hernienbezogene Faktoren (COPD, Diabetes mellitus, morbide Adipositas, Nikotinabusus, Antikoagulanzien und Thrombozytenaggregationshemmer, vorherige Operationen im Mittel- und Unterbauch, Peritonealdialyse, Leberzirrhose und Aszites), chirurgenbezogene und anäshesiebezogene Faktoren. Die endoskopischen Reparationstechniken haben auch aus versicherungstechnischen Gründen in den Verreinigten Staaten keine große Popularität erlangt.
