Osteoporose – Epidemiologie und Versorgungsqualität

Zusammenfassung

Der vorliegende Review gibt einen Überblick über aktuelle epidemiologische Daten zur Osteoporose sowie den Stand der Versorgungsqualität mit besonderer Berücksichtigung des höheren Lebensalters. Drei Viertel aller Patienten mit vertebralen und peripheren Frakturen haben das 65. Lebensjahr bereits überschritten. Bei den Hüftfrakturen sind 3 von 4 Patienten älter als 70 Jahre. Die Assoziation zwischen Alter und Frakturen resultiert aus der fast ausnahmslos zugrunde liegenden Osteoporose sowie den häufigeren Stürzen im höheren Alter.
Die Prävalenz der Osteoporose wird in Deutschland für Frauen über 50 mit 24 % angegeben; bei Männern beträgt diese 6 %. Betrachtet man die geschlechtsspezifische Inzidenz der Hüftfrakturen, so findet sich bei den Frauen eher ein Rückgang, während beim männlichen Geschlecht die Inzidenz weiteransteigt. Trotz des Rückgangs der Inzidenz bei Frauen kommt es durch den demografischen Wandel zu einer absoluten Zunahme bei beiden Geschlechtern mit erheblichen gesundheitsökonomischen Folgen.
Trotz dieser Entwicklungen ist die Behandlungsrate von Patienten mit Osteoporose oder bereits erlittenen Fragilitätsfrakturen seit Jahren konstant gering. Im Gegensatz dazu haben sowohl nationale als auch internationale Studien gezeigt, dass durch verschiedene Maßnahmen, z. B. einen „fracture liaison service“, die Adhärenz der Patienten vorausgesetzt, die Rate der Folgefrakturen um bis zu 50 % gesenkt werden könnte.
