Notfallmedizinische Versorgung von Patienten mit Kunstherz

Zusammenfassung

In den letzten Jahren hat sich das Missverhältnis zwischen Organspendern einerseits und Patienten auf der Warteliste zur Herztransplantation andererseits weiter verschärft. Ohne die zunehmende Implantation von Kunstherzsystemen zur Überbrückung würde ein signifikanter Anteil von Patienten, bedingt durch die lange Wartezeit, nicht überleben. Neben diesem sog. Bridge-to-Transplant-Konzept werden aber auch zunehmend Patienten mit dem Ziel einer „Destination Therapy“ versorgt. Dieses Konzept kommt für Patienten mit einer Kontraindikation zur Herztransplantation in Frage und bedingt die Versorgung mit einem Unterstützungssystem bis zum Lebensende. Die aktuell verfügbaren Kunstherzsysteme sind in den letzten Jahrzehnten soweit gereift, dass eine ambulante Führung des Patienten in den meisten Fällen möglich ist. Dementsprechend steigt die Wahrscheinlichkeit prähospitaler Kontakte medizinischer Erstversorger mit diesen häufig komplexen und mit zahlreichen Komorbiditäten belasteten Patienten. Damit einhergehend kommt es zur Konfrontation mit einer Vielzahl technischer Systeme und deren Bedienung, welches zu Unsicherheit bei der Durchführung der notfallmedizinischen Versorgung führen kann. In diesem Artikel wollen wir einen Überblick über die häufig implantierten Systeme, deren häufigste Komplikationen im intensiv- und notfallmedizinischen Bereich und die praktische Herangehensweise an einen Kunstherzpatienten im Notfall geben, insbesondere hinsichtlich der Durchführung von BLS („basic life support“) und ACLS („advanced cardiac life support“).
