Mini-Mental-Status-Test im stationären geriatrischen Bereich

Eine Evaluation der diagnostischen Qualität

Zusammenfassung

Hintergrund

Bisherige Studien ermittelten anhand von Testgütekriterien eine Überlegenheit alternativer Demenz-Screening-Tests gegenüber dem Mini‐Mental-Status-Test (MMST). Ziel der in dieser Arbeit durchgeführten Untersuchung ist die Evaluation des MMST speziell im stationären medizinisch‐geriatrischen Bereich.

Patienten und Methoden

Mit 154 Patienten einer geriatrischen Klinik wurden der MMST und der DemTect-Test durchgeführt. Auf Grundlage der Ergebnisse im DemTect wurden 71 Personen als kognitiv unbeeinträchtigt und 83 Personen als kognitiv beeinträchtigt eingeschätzt.

Ergebnisse

Nach dem Multi‐Trait‐Multi‐Method-Ansatz weist der MMST eine ähnliche konvergente und divergente Validität wie der DemTect auf, obwohl die Tests nur mittelmäßig korrelieren. Der DemTect klassifiziert mehr Patienten als kognitiv beeinträchtigt als der MMST. Der MMST erreicht eine geringe bis mittlere Reliabilität (interne Konsistenz), wobei die Trennschärfe und die Schwierigkeitsindizes der meisten Aufgaben nicht das Optimum erreichen.

Schlussfolgerung

Insgesamt sind der MMST und der DemTect nicht gegenseitig ersetzbar. Die durchschnittliche kognitive Beeinträchtigung der untersuchten Patienten wird vom MMST als deutlich weniger ausgeprägt eingeschätzt als vom DemTect. Der MMST ist nicht der favorisierte Test zur Erkennung leichter kognitiver Beeinträchtigungen.
