Medikamente sind ein zentraler Bestandteil der medizinischen Therapie. Ein grundsätzliches und bis heute nicht zufriedenstellend gelöstes Problem ist die Situation der Polypharmazie. Sie tritt häufig, aber nicht ausschließlich, im Kontext einer Multimorbidität auf [1], sodass sich mitunter der Gedanke einer Polypragmasie aufdrängt. Häufig ist zu beobachten, dass die einzelnen Disziplinen in der Medizin im Versuch einer maximalen Therapie und eines angenommenen maximalen Outcomes scheinbar unkritisch Empfehlungen aus Leitlinien anwenden, die das Risiko eines negativen Outcomes eher erhöhen statt vermindern [2, 3, 4]. Die Problematik der Extrapolation von Studienergebnissen auf nichtuntersuchte Populationen [5] sowie auch die Forderung nach Einschluss alter und multimorbider Patienten in klinischen Studien [6, 7] sind wohl bekannt. Eine bisher inadäquate Berücksichtigung bei der Erstellung von Leitlinien hat der Umstand gefunden, dass häufig Forschungsergebnisse nicht publiziert werden und so die vorhandene Evidenz zusätzlich verzerrt ist [8]. …
