„Medical emergency team” und Reanimationsteam

Brauchen wir beides?

Zusammenfassung

Intrahospitale Herz-Kreislauf-Stillstände kündigen sich in vielen Fällen durch die Verschlechterung von einfachen Vitalparametern schon Stunden vor dem eigentlichen Ereignis an. Daher wird der Früherkennung und Prävention ein hoher Stellenwert eingeräumt. Zur Institutionalisierung eines frühzeitigen Erkennens wurden in den letzten Jahren, insbesondere im angloamerikanischen Raum und Australien, entsprechende Teams eingerichtet. Je nach personeller Zusammensetzung und Organisationsprinzip werden sie z. B. als „medical emergency team“ (MET) oder „critical care outreach team“ (CCOT) bezeichnet.
Ob durch diese Präventionsteams („rapid response team“, RRT) eine Verbesserung der Krankenhaussterblichkeit zu bewirken ist, wird kontrovers diskutiert. Der vorliegende Artikel soll daher die aktuelle wissenschaftliche Evidenz für die Effektivität dieser Teams näher beleuchten und Entscheidungshilfen geben, ob sich ein Krankenhaus für ein entsprechendes System entscheiden sollte.
