Mechanische freiheitsentziehende Maßnahmen im Krankenhaus

Ein systematischer Literaturüberblick

Zusammenfassung

Die Anwendung mechanischer freiheitsentziehender Maßnahmen (FEM) in der Pflege wurde in den letzten Jahren vermehrt öffentlich diskutiert. Während für den Bereich der Alten- und Pflegeheime inzwischen verlässliche epidemiologische Daten vorliegen, ist für Krankenhäuser weitgehend unklar, wie häufig diese Maßnahmen zur Anwendung kommen. Ziel dieses Übersichtsartikels ist die Identifikation und kritische Würdigung von Prävalenz- und Inzidenzstudien zu mechanischen FEM in Krankenhäusern. Die systematische Suche in den Datenbanken CINAHL, Embase und Medline ergab 72 Treffer, von denen 8 eingeschlossen werden konnten. Nur 4 Studien führen eine explizite FEM-Definition an. Diese Definitionen fallen unterschiedlich aus, Bettgitter z. B. werden nur in 2 Studien berücksichtigt. Die Methoden der Datenerhebung beinhalten Interviews mit Pflegenden, Dokumentenanalyse und direkte Beobachtung. Nur 4 Studien begründen bzw. kalkulieren die Stichprobe. In 2 der 8 Studien wird über nachvollziehbare Schritte zur Vermeidung methodischer Verzerrungen berichtet. Insgesamt ist die methodische Qualität der analysierten Studien eher limitiert. Nur eine kleinere Studie präsentiert u. a. Daten aus deutschen Krankenhäusern. Zukünftige epidemiologische Studien sollten die international geforderten methodischen Qualitätskriterien erfüllen.
