Zusammenfassung

Fragestellung Eine zentrale Fettansammlung ist mit negativen Gesundheitsrisiken verbunden. Dies trifft sowohl im Erwachsenenalter als auch im Kindes- und Jugendalter zu. Es werden einfache anthropometrische Messmethoden benötigt, welche es ermöglichen, Kinder mit erhöhter abdominaler Fettansammlung eindeutig zu identifizieren. Häufig wird der Body-Mass-Index (BMI) genutzt, um die Körperfettmasse von Kindern oder Erwachsenen abzuschätzen. Für die Beurteilung der zentralen Fettmasse eignet sich jedoch der Taillenumfang besser. Das Ziel dieser Studie war es, Perzentilkurven für 6 - 18-jährige Jenaer Kinder zu entwickeln und die Eignung des Taillenumfanges und des BMIs zur Erkennung einer Adipositas zu vergleichen. Methoden In den Jahren 2005/2006 wurden die Körperhöhe, das Körpergewicht und der Taillenumfang (gemessen auf Höhe der größten Einziehung zwischen Rippenbogen und Darmbeinkamm) von 2544 Kindern (1288 Jungen und 1256 Mädchen) im Alter von 6 bis 18 Jahren erfasst. Die Perzentilkurven wurden mittels LMS-Methode erstellt. Ergebnisse Taillenumfang und BMI weisen hohe Korrelationen (Spearman) auf. Der mittlere Taillenumfang nimmt in beiden Geschlechtern mit dem Alter zu, dabei sind die Werte der Jungen durchgehend höher als die der Mädchen. Die Perzentilkurven der Jungen steigen im Altersgang kontinuierlich an, während sich die Kurven der Mädchen ab dem Pubertätsalter nur noch geringfügig verändern. Der Vergleich von BMI und Taillenumfang zur Beschreibung der Adipositas macht deutlich, dass bei alleiniger Verwendung des BMI ein größerer Anteil von Kindern mit erhöhtem Taillenumfang nicht identifiziert wird, während nur wenige Kinder mit einem normalen Taillenumfang einen erhöhten BMI haben. Diese Befunde weisen darauf hin, dass der Taillenumfang zur Erfassung der abdominalen Fettansammlung und damit zur Erkennung von Kindern mit einem erhöhten Erkrankungsrisiko unabdingbar ist. Schlussfolgerung Die Messung des Taillenumfangs sollte in Gesundheitsuntersuchungen routinemäßig Anwendung finden.

Abstract

Purpose A central fat accumulation correlates with adverse health outcomes even in childhood. Therefore simple anthropometric measurements are required which correctly identify children with high trunk fat mass. Mostly the body mass index is used to identify total body fat of a child or an adult, but for the central fat mass the waist circumference is more suitable. The purpose of this study was to develop age- and sex-specific percentile curves for 6- to 18-year old Jena children and to compare the adequacy of circumference and BMI as measures of obesity. Methods Height, weight and waist circumference (at the level of maximum waist narrowing between the tenth rib and the iliac crest) were measured in 2544 children (1288 male and 1256 female) between 6 and 18 years of age in 2005/2006. Smoothed percentile curves were performed using the LMS method. Results High correlations (Spearman) could be found between waist circumference and BMI. The mean waist circumference increases with age in both sexes. By doing so the values of boys are constantly higher than those of girls. The percentile curves in boys continuously increase with age whereas the curves in girls begin to level off with the age of puberty. Our comparison of BMI and waist circumference for the description of obesity shows, that by using BMI alone a greater proportion of children with high waist circumferences could not be identified whereas only few children exist with normal waist circumferences but high BMI values. These findings indicate that, waist circumference is necessary to characterise the abdominal fat deposition and the risk of further morbidity in children. Conclusion The waist circumference should be used routinely as a screening instrument in health studies.
