Präklinisches Airway-Management bei Patienten mit Einklemmungstrauma

Zusammenfassung

Die endotracheale Intubation von eingeklemmten Traumapatienten, die aufgrund ihres Verletzungsmusters beatmet werden müssen, ist erwartungsgemäß erschwert. Daher muss der Notarzt ein Verfahren zur Atemwegssicherung anwenden, mit dem er vertraut ist. Goldstandard ist die konventionelle Intubation mit dem Laryngoskop. Ist dieser Zugang nicht möglich, müssen andere Techniken, wie die ventrale Intubation, die blinde Intubation, die Larynxmaske, die retrograde Intubation oder die Notfallkoniotomie zur Anwendung kommen. 31 % von 162 eingeklemmten Traumapatienten mussten noch im eingeklemmten Zustand endotracheal intubiert werden. In über 4000 Luftrettungseinsätzen unseres Zentrums musste ein eingeklemmter Traumapatient koniotomiert werden. Ein Patient wurde von ventral intubiert. Bei Durchsicht der relevanten Literatur scheint sich die Gesamtzahl der eingeklemmten Traumapatienten, die mittels eines speziellen Verfahrens intubiert werden müssen, auf Einzelfälle zu beschränken. Daher müssen sich Notärzte in der Technik von Spezialverfahren wie der notfallmäßigen Koniotomie durch regelmäßiges Training in Übung halten.

Preclinical Airway Management in Patients with Entrapment Trauma

Endotracheal intubation is expected to be impaired in entrapped trauma patients who have to undergo artificial ventilation due to their injury. Therefore, the rescue physician has to choose an airway access he is familiar with. Conventional intubation by use of the laryngoscope is the golden standard in this case. However, this access might be impossible. Other techniques include ventral intubation, blind endotracheal intubation, laryngeal mask airway, retrograde intubation, and emergency cricothyrotomy. 31 % of 162 patients with entrapment trauma had to be intubated while still being entrapped. In more than 4000 air rescue missions of our center one entrapped trauma patient had to undergo emergency cricothyrotomy. One patient was intubated from ventral. According to pertinent literature the number of entrapped trauma patients that have to be intubated by special techniques does not seem to exceed anecdotal numbers. Therefore, emergency physicians have to maintain skills in special techniques such as emergency cricothyrotomy by repeated training.
