Zusammenfassung
Es wird von einem Notarzteinsatz bei einem 73-jährigen Musiker berichtet, welcher zu Hause β-blockiert im septischen Schock angetroffen wurde. Der neurologische Zustand des Patienten ließ eine korrekte Anamnese nicht zu, weshalb der Notarzt die Verdachtsdiagnose „septischer Schock” nicht anhand der Herzfrequenz des β-blockierten Patienten, sondern aufgrund des klinischen Erscheinungsbildes stellte. Der β-Blocker, welcher die Diagnose erschwerte, hat möglicherweise zum mittelfristigen Überleben des Patienten beigesteuert.
Abstract
We report of a 73 years old musician under therapy with adrenergic β-antagonists, seen by an emergency physician because of neurological deterioration due to a prevalent septic shock. The preclinical diagnosis could not been made with the classical combination of hypotension and tachycardia. However, adrenergic β-antagonists probably had a favourable effect on the patient’s survival in the medium term.
