Zusammenfassung

Die Sicherung des Atemweges gehört zu den Kernkompetenzen des Anästhesisten. Neue Videolaryngoskope sollen die Intubation unter erschwerten Bedingungen erleichtern. In diesem Beitrag werden die Vor– und Nachteile der Videolaryngoskopie diskutiert und es werden einige derzeit auf dem Markt erhältliche Videolaryngoskope vorgestellt.

Abstract

Difficulties in tracheal intubation are the major causes of anaesthesia related morbidity and mortality. Novel videolaryngoscopes facilitate endotracheal intubation by indirect visualisation of glottic structures through optical systems. In this review, differences and the advantages and disadvantages of videolaryngoscopes are discussed.

  • Die Sicherung des Atemweges stellt eine der Kernkompetenzen des Anästhesisten dar. Videolaryngoskope erleichtern das Management des schwierigen Atemweges und sind der direkten Laryngoskopie überlegen.

  • Die Videolaryngoskopie findet in den Leitlinien der DGAI für das Management des schwierigen Atemweges Erwähnung.

  • Videolaryngoskope sind meist wieder verwendbar und verfügen in der Regel über eine Kamera an der Spatelspitze, die den Blick auf den Larynx auf einen externen Monitor überträgt.

  • Im Gegensatz zur direkten Laryngoskopie muss zur Visualisierung der Glottis die oropharyngo–laryngeale Achse nicht eingestellt werden.

  • Videolaryngoskope werden über die Mittellinie der Zunge eingeführt und die Spatelspitze bei den meisten Geräten in der Vallecula positioniert. Die Intubation der Trachea erfolgt unter Monitorkontrolle.

  • Zu den derzeit erhältlichen Videolaryngoskopen gehören das Airway Scope, das Airtraq, das Glidescope, das Macintosh–Videolaryngoskop und das McGrath.

  • Klinische Studien zeigen, dass Videolaryngoskope die Visualisierung laryngealer Strukturen nach Cormack und Lehane im Vergleich mit der direkten Laryngoskopie verbessern.

  • Die Lernkurven für die endotracheale Intubation sind bei Videolaryngoskopen steiler als bei der direkten Laryngoskopie. Daher eignen sie sich auch für die Intubation durch ungeübtes Personal.

  • Die meisten Videolaryngoskope sind netzunabhängig und daher auch für die präklinische Notfallmedizin geeignet.
