Introduction
Nous avons, par une enquête, évalué la prévalence et les pratiques de prise en charge des plaies chroniques dans 14 établissements de soins de suite et de réadaptation (SSR) et soins de longue durée (SLD) du Haut-Rhin et apprécié l’intérêt des médecins et du personnel infirmier quant à la création d’une « Équipe Mobile Plaies et Cicatrisation » (EMPC).
Méthodes
Notre enquête, anonyme et de type transversal, s’appuyait sur les résultats de deux questionnaires, l’un concernant le malade, l’autre le personnel médical et infirmier.
Résultats
Quatre-vingt-seize malades sur les 1 163 hospitalisés au moment de notre enquête répondaient aux critères d’inclusion. La prévalence globale des plaies était de 8,3 p. 100, celle des escarres 6,4 p. 100 et celle des ulcères de jambe 1,6 p. 100. Aucun cas de plaie sur pied diabétique n’a été recensé. La population étudiée se caractérisait par un sex-ratio H/F de 0,37, une moyenne d’âge de 86 ans pour les femmes et 76 ans pour les hommes. La durée d’évolution moyenne des escarres était de 6 mois et de 14 mois pour les ulcères de jambe avec une récidive dans 36 p. 100 des cas d’escarres et 52,6 p. 100 des cas d’ulcères de jambe. L’étiologie des ulcères n’était pas définie dans près de la moitié des cas. Des prélèvements bactériologiques avaient été effectués dans sept cas. Le nettoyage des plaies se faisait le plus souvent avec du savon et du sérum physiologique. Une détersion mécanique avait été pratiquée dans quatre cas. Les hydrocolloïdes étaient les pansements les plus prescrits dans la prise en charge des escarres, alors que les interfaces étaient les plus utilisées pour les ulcères de jambe. Aucune prescription d’antibiotique topique n’a été relevée. Une échelle d’évaluation de la douleur n’avait été utilisée que dans 18 cas, et une anesthésie de surface dans un cas, avant la détersion d’un ulcère de jambe. Une échelle d’évaluation du risque d’escarre avait été remplie pour 27 des 75 malades atteints d’escarres et deux tiers d’entre eux étaient alités sur des matelas de type préventif. Douze des 19 malades atteints d’ulcères de jambe portaient des bandes de compression quotidiennement. La majorité des médecins jugeait bonnes ses connaissances dans le domaine des plaies chroniques, contrairement aux infirmiers qui dans 72 p. 100 des cas les estimaient moyennes voire insuffisantes. Tout le personnel infirmier et 11 des 13 médecins étaient intéressés par la venue d’une équipe spécialisée pour les cas difficiles.
Discussion
Les principales pratiques en adéquation avec les recommandations étaient l’emploi de pansements modernes, bien que leur renouvellement semble trop fréquent, l’utilisation anecdotique de solutions antiseptiques, l’abandon des antibiotiques topiques, la réalisation d’un bilan nutritionnel. Deux tiers des malades atteints d’ulcère de jambe portaient des bandes de compression. En revanche, l’application d’anesthésiant de surface, la détersion manuelle, l’utilisation de grilles d’évaluation de la douleur et du risque d’escarre, la réalisation du bilan étiologique des ulcères de jambes restent à optimiser.
Conclusion
Cette enquête, menée préalablement à la création d’une « Équipe Mobile Plaies et Cicatrisations » basée aux Hôpitaux Civils de Colmar, a pu mettre en évidence la forte motivation des médecins et du personnel infirmier pour bénéficier d’une formation spécifique ainsi que de conseils pratiques pour la prise en charge des plaies chroniques. Elle a permis de mieux cibler les besoins éducatifs des établissements de SSR et SLD dans le domaine de la prise en charge des plaies chroniques.
Summary
Background
We carried out a survey to assess the prevalence of various therapeutic approaches for chronic wounds in 14 primary care and rehabilitation units (SSR) and long-term care units (SLD) in the Haut-Rhin department of France, and we attempted to gauge the interest of doctors and nursing staff in the creation of a Mobile Wound and Healing Unit (EMPC).
Methods
Our anonymous transversal survey was based on the results of two questionnaires, one concerning patients and the other concerning medical and nursing staff.
Results
96 of the 1 163 patients hospitalised at the time of our survey met the inclusion criteria. The global prevalence of sores was 8.3%, while that of bedsores was 6.4% and that of leg ulcers was 1.6%. There were no cases of wounds on diabetic feet. The study population was characterised by a M/F sex-ratio of 0.37, with mean age of 86 years for women and 76 years for men. The mean duration of bedsores was 6 months, compared with 14 months for leg ulcers and a relapse rate of 36% for bedsores and 52.6% for leg ulcers. In more than half of all cases the aetiology of the ulcers was not stated. Bacteriological samples were obtained in 7 cases. Wounds were generally cleansed using soap and physiological saline, with mechanical debridement being used in 4 cases. Hydrocolloids were the dressings used most widely for bedsores, while interfaces were most commonly used in leg ulcers. No topical antibiotics were prescribed. A pain evaluation scale was used in only 18 cases and topical anaesthetics were administered in one case prior to debridement of a leg ulcer. A bedsore risk evaluation scale was completed for 27 of the 75 of the patients presenting bedsores and special preventive mattresses were used for two-thirds of these patients. Twelve of 19 patients with leg ulcers had compression bandaging that was changed daily. Most doctors considered their knowledge of chronic wounds to be good, in contrast with nursing staff, 72% of whom judged their knowledge mediocre or insufficient. All the nursing staff and 11 of the 13 doctors expressed interest in the use of a specialised team for difficult cases.
Discussion
The main practices consistent with the recommendations were use of modern dressings, although the latter appeared to be changed too frequently, anecdotal use of antiseptic solutions, abandonment of use of topical antibiotics and nutrition management plans. Two-thirds of patients with leg ulcers wore compression bandages. However, improvements remain to be made concerning the use of topical anaesthetics, manual debridement, use of pain evaluation and bedsore risk scales, and assessment of the aetiology of leg ulcers.
Conclusion
This survey, conducted prior to the creation of a mobile wound and healing unit based at the Colmar General Hospital, showed that doctors and nursing staff are extremely keen on the idea of specific training and practical advice concerning chronic wound management. It provided a clearer vision of the training requirements of SSR and SLD establishments in terms of chronic wound management.

Interest and difficulties in setting up pharmaceutical reconciliations for patients with dressings for complex wounds

L’implication du pharmacien dans la gestion des pansements complexes peut permettre grâce à la conciliation de renforcer la continuité de cette prise en charge. L’objectif de cette étude était d’améliorer la qualité des informations transmises entre la ville et l’hôpital concernant les pansements de plaies complexes.
Il s’agit d’une étude prospective dans trois services (médecine polyvalente, diabétologie, chirurgie vasculaire) composée de deux groupes : le groupe témoin correspondait à une prise en charge classique des patients, et le groupe intervention à une période avec réalisation de conciliation pansement par le pharmacien. Un suivi des patients après leur retour au domicile a été réalisé auprès des professionnels de ville impliqués.
Vingt patients ont été inclus dans le groupe témoin et 19 dans le groupe intervention. La conciliation d’entrée a permis d’améliorer la qualité et la quantité des informations sur les plaies transmises entre la ville et l’hôpital. La conciliation de sortie a permis de passer de 60 à 100 % de prescriptions de sortie plaie et pansement. Cent pour cent des IDE libéraux interrogés étaient satisfaits de la prise en charge des patients à la sortie.
La conciliation permettrait d’améliorer les informations transmises entre la ville et l’hôpital et d’éviter une rupture dans la continuité des soins de plaies complexes. Cependant, des difficultés ont été rencontrées notamment l’adhésion des services et le temps dédié. Cette étude est la première mettant en évidence l’intérêt de la conciliation DM et devrait être développée à d’autres DM.
The management of wounds is a commonplace activity in a patient's healthcare pathway. The involvement of the pharmacist in the management of complex dressings can help strengthen the continuity of this care thanks to the pharmaceutical reconciliation. The objective of this study was to improve the quality of information transmitted between the city and the hospital regarding complex wound dressings.
This is a prospective study consisting of two groups in three services (medicine, diabetology, vascular surgery): the control group corresponded to a classic patient care and in the intervention group, the pharmacists performed dressing reconciliations. A follow-up of the patients after coming back home was realized with healthcare professionals of city involved.
Twenty patients were included in the control group and 19 in the intervention group. Entry conciliation has improved the quality and quantity of information on wounds transmitted between the city and the hospital. Exit conciliation has increased from 60 to 100% wound and dressing output prescriptions. One hundred percent of nurses surveyed were satisfied with the patients care.
Reconciliation would improve information transmitted between the city and the hospital and avoid a break in the continuity of complex wounds cares. However, the time dedicated and the adhesion of the care services were difficulties encountered. This study is the first highlighting the interest of medical device reconciliation and could allow reconciliation extension toward other medical devices.
Wound dressing in dermatology: Its pain and recognition
2019, DouleursLes pansements peuvent être douloureux, or il s’agit de la thérapeutique primordiale des ulcères. Les douleurs induites par les soins (DIS) sont une priorité. L’étude vise à évaluer la qualité de la prise en charge médicale des DIS par le regard du patient.
Étude monocentrique, hospitalière, en Île-de-France, prospective, transversale, observationnelle, durant le mois d’avril 2017. Le questionnaire était destiné aux patients relevant d’un soin de pansement.
Quatre-vingt-treize patients inclus, d’âge moyen 75 ans. Dans l’attente du pansement, 32,6 % des patients sont « angoissés, anxieux et craintifs ». Cinquante huit pourcent n’avaient pas de prescription de prémédication avant pansement. Après réfection du pansement, 43 patients s’autoévaluent sur l’échelle numérique (EN) à plus de 4/10. Cinquante quatre pour cent des 37 patients ayant reçu une prémédication s’évaluent à plus de 4/10 de douleur per-pansement. La détersion et le nettoyage sont les plus douloureux. Parmi les « insatisfaits et très insatisfaits », 4 avaient une douleur cotée à plus de 7/10 sur l’EN. Les patients critiquent l’évaluation de la douleur et la sous-utilisation des antalgiques.
L’ulcère est une pathologie atteignant des populations vulnérables. Les prémédications sont peu prescrites, et le cas échéant insuffisantes, entraînant une mise en difficulté des soignants et possiblement un retard de cicatrisation.
Wound dressings can be painful, but it is the primary therapy of ulcers. Care-induced pain (CIP) is a priority. The study aims to assess the quality of the medical management of CIP by patients.
Monocentric, hospital in Île-de-France, prospective, transversal, observational, during the month of April 2017. Questionnaire for patients who need wound dressing.
Ninety-three patients included, mean age 75 years. Waiting for the wound dressing, 32.6% of patients were “anxious and fearful”. Fifty eight percent had no pre-medication prescription. After reconstruction of the dressing, 43 patients self-assessed more than 4/10 on the numeric rating scale (NRS). Fifty-four percent of the 37 patients who received premedication rate their pain themselves on the NRS more than 4/10. The most painful nursing is detersion and cleaning the ulcer. Among the “dissatisfied and very dissatisfied”, 4 had a pain rated over 7/10 on the NRS. Patients criticized pain assessment and underuse of analgesics.
The ulcer is a pathology reaching vulnerable populations. Premedication are less prescribed and, if there are prescribed, they are often insufficient, leading to difficulty nurse and delayed healing.
Prevalence and influencing factors of chronic wounds among clients of home care services in Germany
2019, Zeitschrift fur Evidenz, Fortbildung und Qualitat im GesundheitswesenChronische oder nicht heilende Wunden stellen für alle Beteiligten und das Gesundheitssystem ein ernstzunehmendes Problem dar. Aktuell gibt es kaum belastbare Zahlen zur Prävalenz chronischer Wunden in der ambulanten Pflege, obwohl dieses Setting zunehmend an Bedeutung gewinnt. Ziel dieser Untersuchung war es daher, die Prävalenz von chronischen Wunden und deren Einflussfaktoren bei Klienten zu bestimmen, die durch ambulante Pflegedienste versorgt werden.
Im Rahmen einer deutschlandweiten Querschnittserhebung wurden im Jahr 2012 Pflegeempfänger untersucht, die durch ambulante Dienste versorgt werden. Die Stichprobenziehung erfolgte in zwei Stufen. Zunächst wurde je Bundesland eine Zufallsstichprobe ambulanter Dienste gezogen, anschließend erfolgte je Dienst eine zufällige Auswahl von zu befragenden Klienten. Im Rahmen der Untersuchung sollten die Prävalenz chronischer Wunden und beeinflussende Faktoren wie BMI, Alter, Wohnsituation, Aktivität und Diabetes mellitus beschrieben werden. Die Erhebung der Daten mittels Fragebogen wurde von geschulten Pflegefachkräften durchgeführt.
Insgesamt hatten 144 ambulante Pflegedienste mit 1296 Klienten zugestimmt, an der Erhebung teilzunehmen. Schließlich nahmen an der Untersuchung 880 Pflegeempfänger aus 100 ambulanten Pflegediensten teil; die Response lag bei 68%. Von allen untersuchten Pflegeempfängern wiesen 101 mindestens eine chronische Wunde auf, die höchsten Prävalenzen zeigten sich für Dekubitus (4,6%) und Ulcus cruris (4,0%). Pflegeempfänger mit einem Diabetes mellitus waren häufiger von chronischen Wunden betroffen (16,4%) als Pflegeempfänger ohne diese Erkrankung (9,5%). In Gemeinschaft lebende Pflegeempfänger (14,1% chronische Wunden; 6,0% Dekubitus) und Pflegeempfänger unter 65 Jahren (20% chronische Wunden; 9,5% Dekubitus) waren häufiger betroffen als allein lebende Pflegeempfänger (9,1% chronische Wunden; 3,0% Dekubitus) bzw. Pflegeempfänger über 65 Jahren (10,5% chronische Wunden; 4,0% Dekubitus). In der multivariaten Berechnung erwies sich der BMI als stärkster Prädiktor. In der Gruppe der adipösen (BMI >35,3 kg/m2) und in ihrer Aktivität eingeschränkten Pflegeempfänger liegt die Prävalenz von chronischen Wunden bei 44,8%.
Im Hinblick darauf, dass jeder neunte Pflegeempfänger in ambulanten Pflegediensten von mindestens einer chronischen Wunde betroffen ist, sollte hier ein besonderer Fokus auf die Vermeidung, Erkennung und Versorgung von chronischen Wunden gelegt werden. Ein weiterer notwendiger Aspekt ist das Erkennen von Risikogruppen um frühzeitig adäquate und präventive Maßnahmen beginnen zu können.
Chronic or non-healing wounds are a serious problem for both the parties involved and the healthcare system. Currently, there are hardly any reliable data on the prevalence of chronic wounds in outpatient care, although this setting is becoming increasingly important. Therefore, the aim of this study was to determine the prevalence of and the factors associated with chronic wounds in clients cared for by home care services.
As part of a cross-sectional study throughout Germany in 2012, a survey was conducted among care recipients provided by home care services. The sample was drawn in a two-step procedure. First, a random sample of home care services was drawn for each federal state, followed by random selection of clients to be interviewed for each service. The aim of the study was to describe the prevalence of chronic wounds and influencing factors such as BMI, age, housing situation, activity and diabetes mellitus. Data collection through questionnaire was carried out by trained nurses.
A total of 144 home care services with 1,296 clients had agreed to participate. With 880 care recipients from 100 home care services finally taking part in the survey, the response rate was 68 %. Of all care recipients examined, 101 had at least one chronic wound; the highest prevalence rates were observed for pressure ulcers (4.6 %) and leg ulcers (4.0 %). Care recipients with diabetes mellitus were more frequently affected by chronic wounds (16.4 %) than non-diabetic recipients (9.5 %). Community-living care recipients (14.1 % chronic wounds; 6.0 % pressure ulcers) and care recipients under the age of 65 (20 % chronic wounds; 9.5 % pressure ulcers) were affected more frequently than care recipients living alone (9.1 % chronic wounds; 3.0 % pressure ulcers) or care recipients older than 65 years (10.5 % chronic wounds; 4.0 % pressure ulcers). In the multivariate calculation, the BMI proved to be the strongest predictor. The prevalence of chronic wounds in the group of obese (BMI > 35.3 kg/m2) care recipients and those with activity restriction is 44.8 %.
In view of the fact that one out of nine care recipients receiving assistance from home care services is affected by at least one non-healing wound, special attention should be paid to the prevention, detection and treatment of chronic wounds. Furthermore, it seems particularly necessary to identify the relevant risk groups in order to be able to initiate adequate and preventive measures at an early stage.
Coexistence of risk factors for pressure ulcer and severe frailty concerning patients in long-term care units
2014, NPG Neurologie - Psychiatrie - GeriatrieÉvaluation des marqueurs de comorbidité coexistant avec la survenue d’escarres dans une population âgée en unité de soin de longue durée (USLD).
Nous avons établi la prévalence d’escarres chez 153 patients d’USLD et recherché l’existence de marqueurs de comorbidité à un stade sévère : trouble de la microcirculation, déficit immunitaire, dénutrition sévère et fragilité cognitive.
La prévalence des escarres était de 15,7 %. Parmi les marqueurs de comorbidité, seules la dénutrition sévère (33 % vs 10 % ; p < 0,05 %) et l’atteinte sévère de la microcirculation (37,5 % vs 18 % ; p < 0,05 %) étaient des facteurs qui différenciaient les patients ayant des escarres de ceux qui n’en n’avaient pas.
Il est important de prendre en compte ces deux facteurs dans la formation des équipes de soin pour assurer une bonne prise en charge des patients fragiles à risque d’escarres.
Evaluation of comorbidity risk factors and pressure ulcer risk factors among frail independent ederlies in a long-term care unit. We compared the occurrence of risks factors among bearers and non-bearers of pressure ulcers.
We established the pressure ulcer prevalence among 153 patients hospitalized in a long-term care unit. For each, we looked for the existence of severe comorbidity markers: microcirculation impairment, immunodeficiency, severe malnutrition or cognitive impairment.
Pressure ulcer prevalence was 15.7%. Only severe malnutrition (33% vs 10% P < 0.05%) and severe atherosclerosis (37.5% vs 18% P < 0.05) were differentiating factors for the 2 populations.
It is important to take into account these risks factors in the training of care-givers so as to ensure optimal support.
